Searching tool to generate forms

Hi,
I have the following problem to solve. In our application we have the
user to fill data into forms. The forms are currently available on
paper. We want to scan in the paper and place controls on the forms, so
the user can enter that data directly in to the form on screen. After
the data is entered we save it to a sql server db. Later we want to
print the whole form or just the data that was entered onto the
original paper. We can not use word because our customer dont want to
buy world licences for all workstations just because the user have to
fill data into some forms. Is there any tool that can do this. Its
something like crystal reports but with the possiblity to enter data
into reports and send it back to the application.
